This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Wrigley's Rx Pharmacy is required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect.
How we may use and disclose your health information
Treatment
We use your PHI to fill your prescriptions, counsel you about your medications, check for drug interactions, and coordinate your care. For example, we may contact your prescriber to clarify a prescription or to request a refill authorization.
Payment
We use and disclose your PHI to bill and collect payment for the products and services we provide — for example, submitting claims to your insurance plan or pharmacy benefit manager.
Health care operations
We may use your PHI for our internal operations, such as quality review, staff training, licensing, and compliance activities.
Refill reminders and treatment communications
We may contact you to remind you that a prescription is due for a refill, is ready for pickup, or to tell you about treatment alternatives that may be of interest to you, as permitted by law.
Other permitted or required disclosures
- When required by federal, state, or local law
- For public-health activities (for example, adverse-event reporting or immunization registries)
- To health-oversight agencies for audits, inspections, or licensure
- In response to a court order or other lawful process
- To avert a serious threat to health or safety
- To business associates who perform services for us under written agreements that require them to protect your PHI
Uses and disclosures other than those described above — including most uses for marketing, any sale of PHI, and most disclosures of psychotherapy notes — will be made only with your written authorization, which you may revoke at any time.
Your rights regarding your health information
- Access: You may inspect and obtain a copy of your PHI, including an electronic copy, generally within 30 days of your request.
- Amendment: You may ask us to amend PHI that you believe is incorrect or incomplete.
- Accounting of disclosures: You may request a list of certain disclosures we have made of your PHI.
- Restrictions: You may request limits on how we use or disclose your PHI. We must agree to a request to restrict disclosure to your health plan when you pay for an item or service in full, out of pocket.
- Confidential communications: You may ask us to contact you in a specific way or at a specific location.
- Paper copy: You may request a paper copy of this notice at any time, even if you received it electronically.
- Breach notification: We will notify you if a breach occurs that compromises the privacy or security of your unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
Changes to this notice
We reserve the right to change this notice and to make the revised notice effective for PHI we already hold. The current notice is posted in our pharmacy and on this page.
